Your Rights If you have concerns, questions, feedback or a complaint, please talk to your Dr Lee or the receptionist. You may prefer to write or use the feedback form below. Your concerns, suggestions complaints and feedback are taken seriously. However, if you wish to take matters further and feel that you need to discuss the matter outside of the surgery there are several options available. This includes The Medical Registration Board, AMA or Health Care Complaints Commission (HCSCC).
Your Responsibilities Dr Lee and all staff must be treated with respect at all times. Verbal abuse or threats will not be tolerated and will result in your child being discharged from the service.
Your Privacy The Federal Laws relating to your Privacy were amended in 2001. However, because these matters are now formalised in law we must comply with these laws and make some thinks clear to you. There are really two elements you need to be aware of.
1. Freedom of Information
The notes we write and the pathology reports etc remain our property but you have always had access to the contents of these notes. There is an administrative cots. The cost involved are for our time but there are no Medicare rebates for these services. If you visit another doctor, we can send copies of these notes to them, provided you give written permission.
2. Personal Privacy
Your medical file is handled with the utmost respect for your privacy. Our staff are bound by strict confidentiality requirements as a condition of employment. Personal information is handled in accordance with this practice’s privacy policy and consistent with the privacy legislation. Ordinarily we will not release the contents of your medical file without your consent.
As a part of the assessment of your medical condition, a full history is taken. This will include a full medical history, drug history and family history, some of which may not be directly relevant to the current problem. You may elect to withhold information, but this may be to your/your child’s detriment in the future. You may request that some information not be disclosed to other health care professionals. Clinical information is included for the following:
- Requests for blood tests and X-rays
- Referrals to Medical Specialists and allied health professionals
- Letters to the referring doctor
- Medicare and Private Health Funds are given item numbers for billing purposes
